I saw that more often when the tape had been stored sideways in a car or other vibration environment. If it had binding problems I fully fast-forwarded-rewound it before trying again (and sometimes pulled the crinkled part out and pulled it across a wooden pencil to try to straighten it out a bit, although I'm not sure that helped.)
